Billing & Payment
If your insurance requires a referral, it is your responsibility to make sure you have the referral at the time of service. If you do not have your referral this may lead to excessive delays or rescheduling. If you fail to obtain the necessary referral, you will be responsible for the full payment.
CONDITIONS NOT COVERED
There are certain diagnoses that are not covered by your insurance. If we do not participate with your insurance company then you will be responsible for full payment. Please contact your insurance company regarding your benefits.
All co-pays are to be paid at the time of service. If you are unable to pay at that time, we will need to reschedule your appointment.
You will need to bring all insurance cards with you and payment will be requested at the time of service.
